We are pleased you are interested in fostering a homeless animal. The following information is requested so that we can assist you in selecting a new foster dog. The animal’s welfare is our highest priority. This application is designed to help us determine if the fostering is in the animal’s best interest as well as helping you find an animal most compatible with your lifestyle.

Many homeowner’s insurance companies have breed restrictions which may include, but are not limited to; Pit Bulls, American Pit Bull Terriers, American Staffordshire Terriers, Rottweilers, Dobermans, Akitas, American Bulldogs, and Chows. They will not insure you if you reside in your home and may cancel your policy if you add a dog breed that is prohibited. Please check your insurance policy regarding breed discrimination BEFORE submitting an application. Landlords may also have breed restrictions due to their insurance policies.

I understand that omission of the information and/or failure to answer all the questions can result in a denied application. Also, if an omission or untruth is discovered after a fostering commitment has been made, I understand that Adopt a Homeless Animal Rescue, Inc. reserves the right to annul the foster commitment and reclaim the animal. I give Adopt a Homeless Animal Rescue, Inc. permission to fully investigate the information provided as well as contact veterinarians and other related officials. If the application passes review, I agree to a home and yard visit on a mutually agreed upon date before a fostering commitment is made. In addition, I understand the fostering decision is dependent on many factors, including but not limited to the compatibility of the family and home to the individual animal.
